Collecting information
We collect as much personal and personal health information as needed to deliver our public health programs and services effectively.
This may include:
- Ontario health card numbers
- names, addresses and dates of birth
- information related to eligibility or payment for services
- health-related assessments about individual or family health history including exposure to disease

We will tell you why we need any information and what it will be used for. This may be through an in-person meeting, over the phone, or through written or electronic notice.
