Who can book an appointment at an HPPH vaccination clinic

We are currently booking appointments for:

  • Individuals 12 years of age and older
    • Individuals under the age of 18 may only receive the Pfizer-BioNTech vaccine
    • Individuals 18 and older may receive the Pfizer-BioNTech or Moderna vaccine
    • Note: You must be at least 12 years old or older on the day of the clinic to book an appointment
  • We continue to book second dose appointments for Pfizer and Moderna vaccines for those eligible. Please see our COVID-19 Vaccine Information: Second Dose page for details and to find out if you are eligible. 
How to book an appointment
Please note: an appointment is required. Appointments are only available to those who are eligible at this time. Please arrive at your scheduled appointment time, not earlier.
  • Use the booking links on this page, or call 1-833-753-2098. Huron Perth Public Health is not using the provincial booking system at this time.
  • If you are able to book online, or if you are a friend/caregiver and can help someone book online, we are asking that you please book online rather than calling. Our phone lines are very busy.
  • When booking, please double check that your date of birth is accurate and that you have changed the year from 2021 using the drop-down calendar.
  • If you see a confirmation page, your appointment has successfully been booked. 
  • Reminder: Once your appointment has been booked you will receive an email confirmation with your scheduled appointment time. Please check your junk and spam email folders in case the email was delivered there instead of your inbox (Note: it can take up to an hour for this email to send when our booking system is busy). 

Vaccines at HPPH community clinics

  • All vaccines approved by Health Canada are safe and effective. We encourage you to get the type of vaccine that is available to you, when you are eligible to receive the vaccine.
  • Currently, both the Pfizer-BioNTech and Moderna COVID-19 vaccines are being administered at HPPH mass vaccination clinics. All clinics on the website now show which vaccine is expected to be administered.
  • Please note that there are no walk-in, end-of-day doses available at HPPH clinics. HPPH has a process for administering any end-of-day doses; this process involves a list of eligible vaccination candidates. Please do not come to an HPPH clinic if you do not have an appointment, as there will not be an opportunity to receive vaccine.

  • Several pharmacies are offering Pfizer and Moderna COVID-19 vaccines. Please visit COVID-19 Pharmacy Vaccine Locations to learn more.
Mixing vaccine types for your second dose 
  • Receiving the same vaccine for the first and second dose, or a mixed schedule, are both safe and effective options to gain strong protection from COVID-19.
  • Pfizer and Moderna are interchangeable COVID-19 mRNA vaccines. This means that you can receive one of these vaccines for your first dose and the other vaccine for your second dose. Mixing COVID-19 mRNA vaccines provides safe and highly effective protection against the COVID-19 virus, including the variants of concern. 
  • If you would like to receive a specific vaccine product, then book into an HPPH clinic that offers that vaccine. All clinics on the website now show which vaccine is expected to be administered. However, due to supply changes, we cannot always guarantee the availability of a specific product. In that case, we will offer an appropriate alternative. You can then decide if you wish to proceed and receive the vaccine.
  • If you want to rebook into a different clinic, you must first cancel your current appointment.
Cancelling your appointment 
  • If an appointment has been double-booked, or you can no longer make it to your scheduled clinic time, please cancel your appointment to free up the space for another eligible community member.
  • To cancel your appointment, use the online form below, or if additional support is needed, you may contact our booking line at 1-833-753-2098 and press 2 (two). Please include the spelling of your first and last name, and the date, time and location of your scheduled appointment when leaving a voicemail message.

Preparing for your appointment & pre-screening information
  • Do not attend your appointment if you are feeling unwell or have signs and symptoms of COVID-19. You can cancel your appointment using the online cancellation form, or by phoning 1-833-753-2098.
  • If you need assistance, please bring a family member, friend or caregiver with you to your appointment. Unless this person is in one of the eligible groups, they will not be able to receive the vaccine.
  • Bring any device you may use to assist you with mobility.
  • There will be wheelchairs available at our clinics for those who need mobility assistance.
  • Bring your health card with you, if you have one.
  • A mask is required.
  • Wear a short sleeve shirt.
  • Please note: you will be asked to wait 15-30 minutes after your vaccination.
  • Please arrive at your scheduled appointment time, not early. You will be called in based on your appointment time, not first come first serve.

Pre-screening questions

Please consult with your healthcare provider or your allergist/immunologist before your appointment, if you:

  • have received any other vaccine in the past 14 days
  • have had previous allergic reactions to other vaccines, the COVID-19 vaccine, or any ingredients in the COVID-19 vaccines (polyethylene glycol, tromethamine (Moderna only), or polysorbate)
  • have an autoimmune condition, weakened immune system or are taking medications that weaken your immune system (e.g., high dose steroids, chemotherapy)

Inform the person giving you your vaccine, if you: 

  • are or could be pregnant or are breastfeeding
  • have fainted or became dizzy after receiving a past vaccine or medical procedure or you have a fear of needles (so the provider may offer supports to assist you)
  • have a bleeding disorder or are taking medication that could affect blood clotting (to help the healthcare provider prevent bleeding or bruising from the needle)

Tips for keeping calm before and at your appointment

It is normal to be nervous or anxious before your vaccine. You aren’t alone, and our staff are there to support you through the process. If you feel nervous about your COVID-19 vaccination, try playing your CARDs:

  • Comfort: Think ahead about what would make you more comfortable, like wearing cool comfortable clothing.
  • Ask Questions: Talk to someone you trust about vaccinations or strategies for relaxing.
  • Relax: Think about things that help you relax. Try taking some slow deep belly breaths, counting as you breath in and out.
  • Distract: Distract yourself. Listen to music or talk with your vaccinator.

Remember: 

  • Have something to eat and drink some water before your appointment
  • Wear cool comfortable clothing
  • Plan for ways to relax, like bringing your headphones and relax by listening to music
  • If you’re nervous about needles or you have fainted or become dizzy before with immunizations, please let your vaccinator know. We want you to feel comfortable & safe.

It is important to continue following public health measures to prevent the spread of COVID-19. Please continue to stay home when you are sick, avoid close contact with those outside of your household, practice physical distancing, wear a mask, and wash your hands frequently. Monitor for symptoms of COVID-19 and get tested if symptoms develop.

Vaccinated outside of Ontario/Canada

If you have received one or more doses of a COVID-19 vaccine outside of the Province of Ontario, you must provide proof of immunization to Huron Perth Public Health to have your COVID-19 immunization record documented into COVaxON. Please complete the COVID-19 Out-of-Province Dose Documentation form to notify HPPH of your out-of-province dose administration, and upload the documentation you received.

Current clinic dates & locations:

All clinics on the website show which vaccine is expected to be administered at that clinic. We are providing Pfizer or Moderna only. 

  • Pfizer and Moderna are interchangeable COVID-19 mRNA vaccines. This means that individuals can receive either vaccine as their second dose, regardless of which vaccine they received as their first dose. Visit our COVID-19 Vaccine Information: Second Dose page for more information. 
  • If you had an mRNA vaccine (Pfizer or Moderna) as your first dose and are eligible for your second dose, you may choose to book a second dose at any of the following clinics.
  • If you had AstraZeneca as your first dose and are eligible for your second dose, you may choose to either receive an mRNA (Pfizer or Moderna) vaccine for your second dose or book an appointment for a second dose of the AstraZeneca vaccine at a pharmacy. Both of these options are safe and provide strong protection against COVID-19.
  • Due to supply changes, we cannot always guarantee the availability of a specific product. In that case, we will offer an appropriate alternative. You can then decide if you wish to proceed and receive the vaccine. If you want to rebook into a different clinic, you must first cancel your current appointment.

Important: If you've booked a vaccination appointment, your confirmation is coming! If you see the "Your appointment has been booked!" screen, or you are redirected to the HPPH Vaccine Standby List, your appointment has been booked. Please be patient - you should receive your confirmation email from "CheckMarket Notification" within 15 minutes. Remember to check all inboxes, including any junk/spam folders. If you still have not received an email after 15 minutes, contact us at 1-833-753-2098. Please do not book another appointment. It will disrupt the booking system and hamper others being able to book.
