Adverse Effects Following Immunization (AEFI)

Some patients may experience adverse effects, such as an allergic or neurological response, to a vaccine. As a healthcare provider, you are required to report all adverse side effects following immunization to Huron Perth Public Health. This information helps us track vaccine safety and improves Ontario immunization programs.

Report an adverse event

If your patient has an adverse event to any vaccination, report it to Huron Perth Public Health using the Report of Adverse Event Following Immunization (AEFI) form.

Completed AEFI forms can be faxed to:

  • Huron County: 1-844-935-1327
  • Perth County: 519-271-2785

You don't need to report the following common side effects:

  • Fever that isn't accompanied by other symptoms
  • Injection site reactions that last less than 4 days
  • Vasovagal syncope (without injury)
  • Events that are clearly attributed to other causes
