Immunization Records and Reporting

Under the Immunization of School Pupil’s Act, it is a parent's/legal guardian’s responsibility to report their child’s immunizations to public health. 

Your child's healthcare provider and school are not mandated to report your child's vaccinations to public health. In some cases, your healthcare provider may tell you that your child's vaccinations are up to date. However, the information may not have been reported to public health. It is recommended that you obtain a copy of the record and submit it to public health every time your child receives vaccinations.

You can keep track of immunizations on the yellow immunization card. 

Other ways to report immunizations

HPPH also accepts immunization records using the following methods:

Scan or take a picture of your child's immunization record and email it to the Immunization Team at immunization@hpph.ca. We will confirm that we received your email. 

Please note: Email is not confidential. If you are sending personal health information, you may want to report with a different method.

Send a confidential fax to HPPH at 519-271-2785 or toll-free at 1-877-271-2785. Ask your child's primary care provider to send the records directly or you can fax them yourself. 

Bring your child's yellow immunization card to one of the HPPH offices and we can update their immunization records. 

Call the Immunization Team at 1-888-221-2133 ext 3558 to provide the information over the phone. If you are calling after hours, leave us a voice message and we can update your child's records. 

If you do not have an immunization record for your child

If your child had vaccines in the past, contact your current or past healthcare provider(s) and request a copy of your child's vaccination record.

If you moved from another city, you may be able to contact your previous health unit to obtain the records that they have on file.

If your family doctor has retired, contact the College of Physicians and Surgeons of Ontario or call 416-967-2603.

If you do not have any records of your child's vaccinations, it is recommended that your child complete a catch-up schedule. Re-vaccination is not harmful and is recommended. This will ensure that your child is adequately protected against vaccine preventable diseases.

Vaccine requirements

The Immunization of School Pupils Act and the Child Care and Early Years Act (section 35) require all children attending school or a licensed childcare program in Ontario to receive certain vaccines. 

Make sure that your child gets all necessary vaccinations and report each immunization to Huron Perth Public Health. Remember that an out-of-date immunization record could result in your child's suspension from school. 

All required vaccinations are free and are included in Ontario's routine immunization schedule

In keeping with the Immunization of School Pupils Act, your child needs to receive the following vaccines in order to attend school in Ontario: 

  • Measles 

  • Mumps 

  • Rubella 

  • Diphtheria 

  • Tetanus 

  • Polio 

  • Meningococcal 

  • Whooping cough (pertussis) 

  • Chickenpox (varicella) (only applies to those born on or after 2010)

In keeping with the Immunization for Children in Schools and Licensed Child Care Settings Protocol, 2018 [PDF], your child needs to receive the following vaccines if they are in licensed childcare. 

Required vaccines for licensed childcare
Vaccine required Schedule
Diphtheria, pertussis, tetanus, polio and Haemophilus influenzae type b (Pediacel/Pentacel) Primary vaccine series of three doses (usually given at 2, 4 and 6 months), plus booster dose at 18 months.
Pneumococcal conjugate Primary series of three doses usually given at 2, 4 and 12 months.
Varicella (chickenpox)

Two doses required:

  • One dose after the first birthday (recommended at 15 months)
  • Second dose at least three months after first, provided with MMRV at 4-6 year booster

OR bloodwork showing immunity.

Measles, mumps, rubella (MMR) One dose on or after the first birthday.
Meningococcal conjugate-C One dose on or after the first birthday.
Measles, mumps, rubella, varicella (MMRV) One dose given at 4-6 years of age.
Diphtheria, pertussis, tetanus, polio (Adacel-Polio) One dose given at 4-6 years of age.

Immunization Records

Huron Perth Public Health (HPPH) collects, screens and maintains immunization records for all children who attend school or licensed childcare in Huron and Perth counties. 

Children who do not have a complete immunization record with all required vaccines, or a valid exemption, may be suspended from school or childcare. Learn more about valid vaccine exemptions
